New Patients

Registering at the Practice

You do not need to inform your current GP Practice that you wish to move, you simply need to fill in a registration form; you can collect a form from the Practice, or you can download a copy via the link below and bring the completed copy with you when you visit the practice. Alternatively you can complete our online registration form below.

Each person joining in a family will need to complete in a form, fill in as much of it as possible – don’t worry if you do not know your NHS number as it can be found here or we can request it for you.

Once you have completed the form, you will need to bring your form to the practice and deliver it to reception where we can complete the registration process.

You will need to have successfully registered with us before we can offer you any services.

When you register you will be asked to fill in a questionnaire and you will be offered New Patient Health Check.

Please ensure you have enough medication to last a week when you join to allow us time to obtain your medication details from your previous GP.


Practice Boundary

Before you register with the surgery, please ensure you live within our service boundary by entering your postcode below.

Catchment Area

The Practice boundary is broadly:

A19 to the West

A183 to the North

Ormond street, Barnes Park Road, Silksworth Lane, Essen Way, Tunstall Road, Tunstall Hope Road, Burdon Lane to the East

Burdon Lane to the South

(Areas near to but not included in our boundary are: Grangetown, Leechmere, Millfield, Pallion, Pennywell, South Hylton, Ryhope, Seaton and anywhere over the River Wear)


Moving out of our Catchment Area

If you are registered with us and are moving outside of our catchment area (see above), unfortunately you will need to find a new GP nearer your new home.

We will look after your medical needs for 30 days, whilst you find a GP practice near your home, most GP practices have the same joining process as ours.

If you require any medications please make sure you order a sufficient amount, as you may not be able to order medications at your new practice straight away: reception can help you with this.
